2. Instructions for completing forms

Please review the following information and instructions before completing the SSOOPO forms. Forms can be typewritten or completed by hand.

  • 10.03-H: Warning Concerning Attached Stalking Protection Order

If your petition for a SSOOPO is granted, the respondent will be served with a copy of this warning form. This warning advises respondents of the legal penalties associated with violating a SSOOPO.

  • 10.03-G: Instructions For Obtaining a Civil Stalking or Sexually Oriented Offense Protection Order (SSOOPO)

These instructions will assist you in completing your petition.

Forms Needed to File a SSOOPO:

  • Civil Case Information Form

This form is required for all civil cases. The "Main Plaintiff" is the name of the individual that is requesting a SSOOPO. The "Main Defendant" is the name of the individual that is being filed against.

  • 10.03-D: Petition for Civil Stalking or Sexually Oriented Offense Protection Order (SSOOPO)

This form is filed with the Clerk of Courts and will become public record. The "Petitioner" is the name of the individual that is requesting a SSOOPO. The "Respondent" is the name of the individual that is being filed against.

Do not sign this document unless you are in the presence of a Notary Public. Your documents can be notarized at the Courts Building at the time of filing. A valid photo ID is required to notarize documents.

  • 10-A/DR-82 Protection Order Notice to NCIC/Law Enforcement Information

This form will be provided to law enforcement when a protection order is granted. On page 1, complete the top 2 sections and the "List All Protected Persons" segment at the bottom. Leave the middle sections blank. Fill out as much information as possible. If you are unsure of information, please leave those sections blank.

 

Please bring all completed forms to room 103 of the Montgomery County Courts Building. SSOOPO staff will assist you in filing these documents with the Clerk of Courts.
